Print page 1 on both the front and back - 4 - 4.25" x 5.5" cards will print per page. #2 - parental information flyer to pass out as parents register their children. 2 - 5.5" x 8.5" flyers will print per page.

This document contains the parental information and registration/consent card all on one page. Print this document on both sides of the 8.5"x11" paper and cut paper in half to 8.5" x 5.5". You may perforate across the center of the 8.5" x 5.5" page to make easy to use or cut with scissors after the parent has filled out the card.
